Play/B-Active Program Terms & Conditions

On submitting your Play Program Registration, you agree to the following Terms and Conditions:

CONSENT TO PARTICIPATION AND RISK WARNING

I acknowledge that, with any physical activity, there are some risks associated with the program, including, but not limited to, possible injury or property damage. I understand and acknowledge these potential risks and consent to mine or my child’s participation in the program.

PARTICIPANT CONDUCT

To ensure our participants have a safe and enjoyable time, it is essential that you or your child:
• Follows all instructions give to them by a Return Serve Coach, Program Volunteer or School/Community Group staff member.
• Is respectful and courteous to fellow participants, as well as, Return Serve Coaches, Program Volunteers and School staff members.
• Do not behave in a way that might cause harm to themselves, fellow participants, Coaches and Volunteers, School staff members or any property.

I understand that if my child breaches any of the above conduct requirements, they may be asked to temporarily or permanently leave the Return Serve program.

Note: Our Play and B-Active Programs are not a child-care services. You may drop your children off or stay with them (we encourage our parents to come and join in as well!). In the case of an incident, illness or bad behavior, a parent or caregiver contact must be available to come and pick up the child.

CONSENT TO MEDICAL TREATMENT

I authorise any Return Serve Employee or Volunteer to use and disclose mine or my child’s medical information and to obtain on my behalf and at my expense any medical assistance, treatment and transport as deemed reasonably necessary.

PERSONAL INFORMATION

I confirm that all information provided in this form is true and correct, in particular, all medical conditions and relevant medication.

I understand that Return Serve keeps all personal information confidential and I consent to Return Serve only sharing mine and my child’s personal information with the following parties:
• Tennis Australia for insurance purposes.
• Paramedics or Medical Staff in the case of a medical emergency.
